2. Apply for 4 prototypes;

6. Apply for the Chinese inscription of the certified product and the Chinese warning label of the product;
7. Factory inspection report (preliminary review and factory review report), if the initial application is to fill in the factory survey form;
8. Copies of the "Business License of Enterprise Legal Person" of the applicant, manufacturer and production plant;
9. The application form for compulsory product certification shall be filled in according to the applicant's "Business License of Enterprise Legal Person";
10. Appearance photos of each model in the same application form;
11. List of components related to safety and electromagnetic compatibility;
12. The applicant, the manufacturer and the manufacturer must provide relevant agreement certificates at the same time;
13. If the company has not done IS09001 certification, it is also necessary to provide the process flow chart of the certified product.
